Cause: Fall or slip, lack of teamwork and communication
Description of Event: Crossing a swallow pool, the depth suddenly changed. While attempting to grab a slab, I dislocated my middle finger.
Analysis: Lack of communication and teamwork. Several people had passed the same pool before me, and they were standing right there, knowing about the depth change, yet no one warned me.
